🧬 FROM TAILORx TO OPTIMA: A NEW ERA OF PERSONALIZED BREAST CANCER TREATMENT
Can we safely avoid chemotherapy in more patients with early-stage breast cancer?
An important commentary published on October 30, 2026, in the Journal of Clinical Oncology by Drs. Yara Abdou and Lisa Carey highlights how genomic testing continues to transform treatment decisions for hormone receptor–positive (HR+), HER2-negative breast cancer.
🔬 Four landmark clinical trials have shaped this evolution:
✅ MINDACT: Demonstrated that favorable tumor biology can help identify patients who may safely avoid chemotherapy despite clinically high-risk features.
✅ TAILORx: Established that many women with node-negative disease and intermediate Oncotype DX recurrence scores can safely receive endocrine therapy without chemotherapy.
✅ RxPONDER: Demonstrated that menopausal status matters. Many postmenopausal women with 1–3 positive lymph nodes and low recurrence scores can avoid chemotherapy, while premenopausal women demonstrated additional benefit.
✅ OPTIMA (2026): Extended genomic-guided treatment selection to a higher-risk population, including selected patients with up to nine positive lymph nodes.
Using the Prosigna (PAM50) genomic assay, approximately two-thirds of patients in the test-directed group avoided chemotherapy, with the genomic-guided strategy demonstrating noninferior outcomes compared with standard chemoendocrine therapy at approximately four years of median follow-up.
What does this mean for our patients?
We are moving beyond tumor size and lymph node involvement alone toward treatment decisions based on tumor biology, genomic risk, menopausal status, and individual patient characteristics.
Important questions remain, particularly for younger women, patients with greater nodal involvement, and the long-term durability of chemotherapy omission.
As a surgical oncologist and breast surgeon, I believe these findings reinforce the importance of multidisciplinary, personalized breast cancer care.
The goal is not simply to give less treatment. It is to deliver the right treatment to the right patient—avoiding unnecessary toxicity without compromising cancer outcomes.
